本發(fā)明屬于生物醫(yī)學(xué)領(lǐng)域,具體涉及一種肺癌的預(yù)后標(biāo)記物和使用該標(biāo)記預(yù)測(cè)肺癌預(yù)后的方法及應(yīng)用。
背景技術(shù):
肺癌是發(fā)病率和死亡率增長最快、對(duì)人群健康和生命威脅最大的惡性腫瘤之一。近50年來許多國家都報(bào)道肺癌的發(fā)病率和死亡率均明顯增高,男性肺癌發(fā)病率和死亡率均占所有惡性腫瘤的第一位,女性發(fā)病率占第二位,死亡率占第二位。
近年來,隨著大規(guī)模測(cè)序和芯片數(shù)據(jù)的涌現(xiàn),呈現(xiàn)了很多通過分子學(xué)水平預(yù)測(cè)病人預(yù)后的案例。在乳癌中,乳腺癌21基因檢測(cè)(OncotypeDx)是指檢測(cè)乳腺癌腫瘤組織中21個(gè)不同基因的表達(dá)水平,這個(gè)檢測(cè)能夠提供個(gè)體化的治療效果預(yù)測(cè)和10年復(fù)發(fā)風(fēng)險(xiǎn)的預(yù)測(cè)。通過檢測(cè)檢測(cè)21個(gè)基因,觀察他們之間的相互作用來判斷腫瘤特性,從而可預(yù)測(cè)乳腺癌復(fù)發(fā)指數(shù)以及接受化療的效益比。在美國,21基因檢測(cè)已迅速成為乳腺癌治療標(biāo)準(zhǔn)程序,已有超過7500個(gè)醫(yī)生使用多達(dá)65000次以上。截止目前,該檢測(cè)在歐美多個(gè)國家開展已近9年,獲益人群達(dá)15萬。
肺癌是最常見的惡性腫瘤之一,死亡率已高居癌癥死亡率之首。肺癌包括小細(xì)胞肺癌、肺腺癌、肺鱗癌等多種病理類型,其中肺腺癌是最為常見的肺癌類型,其特點(diǎn)為容易出現(xiàn)術(shù)后遠(yuǎn)處轉(zhuǎn)移,惡性程度高、預(yù)后差。得了肺癌究竟能活多久?這個(gè)問題取決于癌癥病理學(xué)類型、腫瘤分期、患者年齡、身體基本情況以及能否得到及時(shí)合理的治療等等因素。肺癌的組織病理學(xué)分型和臨床分期是目前肺癌的臨床預(yù)后指標(biāo),然而肺癌治療反應(yīng)和預(yù)后差別大,因此對(duì)其進(jìn)行分子分型是肺癌個(gè)體化治療的必然要求。在肺癌中,目前還沒有像OncotypeDx這樣的基因檢測(cè)技術(shù)。如果我們也能通過檢測(cè)基因的表達(dá),預(yù)測(cè)病人的預(yù)后,這將為肺癌病人帶來福音。
目前,需要找出肺癌的預(yù)后基因,主要問題有一下幾點(diǎn):(1)大量肺癌樣品的基因表達(dá)數(shù)據(jù)。癌癥病人的樣品不容易拿到,就算拿到樣品,高額的測(cè)序費(fèi)用也是一筆不小的開支。(2)肺癌病人的隨訪數(shù)據(jù)。造成隨訪數(shù)據(jù)丟失的原因主要有部分醫(yī)務(wù)人員對(duì)隨訪不夠重視,患者對(duì)隨訪的不理解,有的患者難以聯(lián)系上。(3)統(tǒng)計(jì)學(xué)模型。生存期的資料一般不服從正態(tài)分布,常用的統(tǒng)計(jì)方法不適用,而要用特殊的統(tǒng)計(jì)方法。此外,隨訪中很多病人的確切生存時(shí)間未知,只知道病人的生存時(shí)間大于某時(shí)間。但這些截尾數(shù)據(jù)不能刪去,因?yàn)檫@不僅損失了資料,而且會(huì)造成偏性。
技術(shù)實(shí)現(xiàn)要素:
本發(fā)明的目的在于提供一種肺癌的預(yù)后標(biāo)記物、使用該標(biāo)記預(yù)測(cè)肺癌預(yù)后的方法及應(yīng)用。
本發(fā)明所采取的技術(shù)方案是:
一種肺癌的預(yù)后標(biāo)記物,所述標(biāo)記物包括以下基因組合:RP11-89K21.1、CDH17、GRIA1、CENPF、AP000438.2、HMMR、RP11-434D9.1、ADH1B、TPX2、OR7E47P,所述基因的序列依次為SEQ ID NO:1~10。
優(yōu)選的,所述標(biāo)記物還包括下列基因中的至少一個(gè)基因:PLK1、HABP2、PBK、GPR37、NEK2、RP11-497G19.2、SLC2A1、PLOD2、ECT2、SPC25,所述基因的序列依次為SEQ ID NO:11~20。
優(yōu)選的,所述標(biāo)記物還包括下列基因中的至少一個(gè)基因:RP5-839B4.8、SKA3、C20orf197、LYPD3、E2F7、CPS1、ABCA3、TICRR、CDKN3、VIPR1、MTL5、FAM64A、SFTPD、BUB1,所述基因的序列依次為SEQ ID NO:21~34。
優(yōu)選的,所述肺癌為肺腺癌。
一種肺癌的預(yù)后預(yù)測(cè)方法,包括以下步驟:
1)從肺癌患者采集樣品,檢測(cè)并獲得肺癌的預(yù)后標(biāo)記物的表達(dá)量;
2)將所獲得的表達(dá)量應(yīng)用于肺癌的預(yù)后預(yù)測(cè)模型,以獲得肺癌預(yù)后風(fēng)險(xiǎn)值;
3)將所獲肺癌預(yù)后風(fēng)險(xiǎn)值與參比值比較,確定所述患者的預(yù)后;
所述肺癌的預(yù)后標(biāo)記物為權(quán)利要求1~4任一項(xiàng)所述;

根據(jù)所獲肺癌預(yù)后風(fēng)險(xiǎn)值與參比值比較,如果小于或等于參比值,被檢測(cè)病人就屬于低風(fēng)險(xiǎn),如果大于參比值,檢測(cè)病人就屬于低風(fēng)險(xiǎn)。
優(yōu)選的,所述肺癌的預(yù)后預(yù)測(cè)模型中COX回歸系數(shù)按以下方法確定:
1)收集的多位肺癌患者關(guān)于肺癌的預(yù)后標(biāo)記物的轉(zhuǎn)錄組數(shù)據(jù)和臨床生存數(shù)據(jù);
2)采用多因素COX模型逐步回歸分析上述肺癌的預(yù)后標(biāo)記物,得COX回歸系數(shù);
所述參比值按以下方法確定:根據(jù)所得COX回歸系數(shù)及上述收集的多位肺癌患者的轉(zhuǎn)錄組數(shù)據(jù),按肺癌的預(yù)后預(yù)測(cè)模型計(jì)算多位肺癌患者肺癌預(yù)后風(fēng)險(xiǎn)值,取多位肺癌患者肺癌預(yù)后風(fēng)險(xiǎn)值的中位值作為參比值。

一種用于評(píng)估肺癌預(yù)后生存期的試劑盒,其包括用于檢測(cè)上述的標(biāo)記物的mRNA或蛋白表達(dá)水平的試劑。
優(yōu)選的,用于檢測(cè)標(biāo)記物的mRNA表達(dá)水平的試劑是與所述基因特異性結(jié)合的引物和/或探針;用于檢測(cè)標(biāo)記物的蛋白表達(dá)水平的試劑是對(duì)所述基因編碼的蛋白特異的抗體。
本發(fā)明的有益效果是:
本發(fā)明克服了肺癌治療反應(yīng)和預(yù)后差別大,導(dǎo)致預(yù)后不準(zhǔn)確定方便的難點(diǎn),通過標(biāo)記物及檢測(cè)標(biāo)記物的表達(dá),迅速、準(zhǔn)確、方便地預(yù)測(cè)了肺癌預(yù)后,從分子水平實(shí)現(xiàn)了肺癌危險(xiǎn)底高低的分級(jí)分層,顯著地將高低風(fēng)險(xiǎn)的患者分開,從而指導(dǎo)個(gè)體化治療,具有較高的臨床應(yīng)用價(jià)值,節(jié)約了醫(yī)療成本。
本發(fā)明找到與肺癌生存相關(guān)的基因,并建立了這些基因和生存時(shí)間之前的數(shù)學(xué)模型。本發(fā)明中的模型做肺癌生存分析,解決了病人的生存時(shí)間有很多截尾數(shù)據(jù),且生存期的資料一般不服從正態(tài)分布的問題。

具體實(shí)施方式
本發(fā)明所選預(yù)后標(biāo)記物及肺癌的預(yù)后預(yù)測(cè)模型的建立均是按以下方法篩選出來的。肺癌的預(yù)后預(yù)測(cè)模型的建立方法,包括以下步驟:
1)收集肺癌患者的轉(zhuǎn)錄組數(shù)據(jù)和臨床生存數(shù)據(jù);
2)采用DESeq分析肺癌病人與正常人之間的基因差異表達(dá),獲得差異表達(dá)基因;
3)采用單因素COX模型分析步驟2)所得差異表達(dá)基因與生存時(shí)間之間的相關(guān)性,篩選出與生存時(shí)間相關(guān)的基因;
4)采用多因素COX模型逐步回歸分析篩選所得基因,得肺癌的預(yù)后預(yù)測(cè)模型。
優(yōu)選的,為保證結(jié)果的準(zhǔn)確性,所述將臨床生存數(shù)據(jù)不完整和總生存時(shí)間小于一個(gè)月的患者數(shù)據(jù)刪除后,再進(jìn)行步驟2)。
下面結(jié)合具體的實(shí)施例對(duì)本發(fā)明作進(jìn)一步的說明,但并不局限于此。
實(shí)施例1
以肺腺癌為例,肺腺癌的預(yù)后預(yù)測(cè)模型的建立:
1)從TCGA下載肺腺癌數(shù)據(jù),包括轉(zhuǎn)錄組數(shù)據(jù)和臨床生存數(shù)據(jù),共515個(gè)樣品;
2)去除臨床數(shù)據(jù)不完整的樣品,如隨訪不完全的數(shù)據(jù),剩余464個(gè)樣品;,本發(fā)明納入的患者的隨訪生存時(shí)間數(shù)據(jù)最早是1986年,最晚是2016年;
3)使用DESeq R包比較正常人和癌癥病人之間的基因表達(dá)量(篩選條件為:|log2FC|>2,padj<0.001),得到1618個(gè)差異表達(dá)基因;DESeq查找差異基因的主要代碼如下:

4)單因素COX并用于分析生存時(shí)間與差異基因的關(guān)系,找出與生存時(shí)間相關(guān)的基因(p<0.001),得到131個(gè)與生存相關(guān)的基因;單因素COX分析代碼如下:

5)對(duì)于單因素分析得到的生存時(shí)間相關(guān)的基因,我們使用多因素COX進(jìn)行分析,采用的是逐步回歸法。最后,我們得到用來評(píng)估肺腺癌生存期的肺癌的預(yù)后預(yù)測(cè)模型;多因素COX分析代碼如下:

6)使用COX分析得到的模型對(duì)每個(gè)樣品計(jì)算風(fēng)險(xiǎn)值,按照風(fēng)險(xiǎn)值的中位值將病人劃分為高風(fēng)險(xiǎn)組和低風(fēng)險(xiǎn)組。使用乘積極限法來估計(jì)生存率及使用Kaplan-Meier (KM)法估計(jì)和繪制生存曲線,并使用log-rank檢驗(yàn)進(jìn)行高低風(fēng)險(xiǎn)組生存曲線的比較,經(jīng)過比較高低風(fēng)險(xiǎn)組兩者差異顯著,從而可以指導(dǎo)臨床醫(yī)生通過檢測(cè)基因的表達(dá)量預(yù)測(cè)病人的生存時(shí)間,提供個(gè)體化治療方案。
實(shí)施例2

使用log-rank檢驗(yàn)對(duì)本實(shí)施例模型進(jìn)行高低風(fēng)險(xiǎn)組生存曲線的比較,結(jié)果見圖1,橫坐標(biāo)代表病人的生存時(shí)間(以年為單位),縱坐標(biāo)代表總生存率。藍(lán)色線條(上面一條)代表低風(fēng)險(xiǎn)組,紅色線條(下面一條)代表高風(fēng)險(xiǎn)組,log-rank檢驗(yàn)顯著性p=0,說明標(biāo)準(zhǔn)模型的高低風(fēng)險(xiǎn)組之間的生存曲線具有顯著的差異。低風(fēng)險(xiǎn)五年生存率為56.4%,高風(fēng)險(xiǎn)五年生存率為18.1%,五年生存率相差明顯。
應(yīng)用ROC曲線圖評(píng)價(jià)本實(shí)施例模型的臨床準(zhǔn)確性,ROC曲線圖見圖2,橫坐標(biāo)是假陽性率(1-特異度),縱坐標(biāo)是真陽性率(靈敏度),計(jì)算標(biāo)準(zhǔn)模型AUC值為0.746,說明模型預(yù)測(cè)結(jié)果準(zhǔn)確性較高。ROC曲線下的面積值在1.0和0.5之間,在AUC>0.5的情況下,AUC越接近于1,說明診斷效果越好。

使用log-rank檢驗(yàn)對(duì)本實(shí)施例模型進(jìn)行高低風(fēng)險(xiǎn)組生存曲線的比較,結(jié)果見圖3, log-rank檢驗(yàn)顯著性p=0,說明標(biāo)準(zhǔn)模型的高低風(fēng)險(xiǎn)組之間的生存曲線具有顯著的差異。低風(fēng)險(xiǎn)五年生存率為60.1%,高風(fēng)險(xiǎn)五年生存率為14.5%,五年生存率相差明顯。
應(yīng)用ROC曲線圖評(píng)價(jià)本實(shí)施例模型的臨床準(zhǔn)確性,ROC曲線圖見圖4,橫坐標(biāo)是假陽性率(1-特異度),縱坐標(biāo)是真陽性率(靈敏度),計(jì)算標(biāo)準(zhǔn)模型AUC值為0.744,說明模型預(yù)測(cè)結(jié)果準(zhǔn)確性較高。ROC曲線下的面積值在1.0和0.5之間,在AUC>0.5的情況下,AUC越接近于1,說明診斷效果越好。

使用log-rank檢驗(yàn)對(duì)標(biāo)準(zhǔn)模型進(jìn)行高低風(fēng)險(xiǎn)組生存曲線的比較,結(jié)果見圖1,橫坐標(biāo)代表病人的生存時(shí)間(以年為單位),縱坐標(biāo)代表總生存率。藍(lán)色線條(上面一條)代表低風(fēng)險(xiǎn)組,紅色線條(下面一條)代表高風(fēng)險(xiǎn)組,log-rank檢驗(yàn)顯著性p=0,說明標(biāo)準(zhǔn)模型的高低風(fēng)險(xiǎn)組之間的生存曲線具有顯著的差異。低風(fēng)險(xiǎn)五年生存率為62.1%(52.3-73.8),高風(fēng)險(xiǎn)五年生存率為11.3%(5.55-22.8),五年生存率相差非常明顯。
應(yīng)用ROC曲線圖評(píng)價(jià)本實(shí)施例模型的臨床準(zhǔn)確性,ROC曲線圖見圖6,橫坐標(biāo)是假陽性率(1-特異度),縱坐標(biāo)是真陽性率(靈敏度),計(jì)算標(biāo)準(zhǔn)模型AUC值為0.804,說明模型預(yù)測(cè)結(jié)果準(zhǔn)確性較高。ROC曲線下的面積值在1.0和0.5之間,在AUC>0.5的情況下,AUC越接近于1,說明診斷效果越好。
本發(fā)明建立的模型是從TCGA下載肺癌病人的rna-seq和臨床數(shù)據(jù),解決了樣品收集難,測(cè)序費(fèi)用高,以及對(duì)病人隨訪的問題。TCGA,即癌癥和腫瘤基因圖譜(Cancer Genome Atlas)計(jì)劃,試圖通過應(yīng)用基因組分析技術(shù),特別是采用大規(guī)模的基因組測(cè)序,將人類全部癌癥的基因組變異圖譜繪制出來。此外,TCGA的隨訪數(shù)據(jù)也做得非常完善,可以很方便地從TCGA下載病人的最新隨訪數(shù)據(jù)。
由于生存期的資料一般不服從正態(tài)分布,且病人的生存時(shí)間有很多截尾數(shù)據(jù),我們選用Cox比例風(fēng)險(xiǎn)回歸模型作為統(tǒng)計(jì)模型。另外,我們采用開源的r 進(jìn)行編程,有利于程序的自動(dòng)化。
